*If you believe you are unsafe and in need of immediate help, please come to our facility immediately or dial 911.

Care Services/Call to schedule an appointment:

This level of care is provided for persons in need of marriage counseling, family counseling, domestic violence and/or substance use assessment and treatment, and all other mental health needs.

If you are coming in for an intake for substance abuse (ASI) or domestic violence (DVI), please bring the following information with you to your appointment so we can complete your assessment:

  • Police Report

You can complete your assessment during regular business hours. You will then be scheduled for your intake appointment.
